Ladies First ("we," "us," or "our") is committed to protecting the privacy of your health information. This Notice of Privacy Practices describes how we may use and disclose your protected health information (PHI) and your rights regarding this information.
We are required by law to:

- Provide you with this Notice of our legal duties and privacy practices
- Follow the terms of the Notice currently in effect
- Notify you if there is a breach of your unsecured PHI
How We May Use and Disclose Your Protected Health Information
Uses and Disclosures That Do Not Require Your Authorization
For Treatment We may use and disclose your PHI to provide, coordinate, or manage your healthcare and related services. For example:
- Sharing information with healthcare providers involved in your care
- Sending prescriptions to pharmacies
- Coordinating follow-up care
For Payment We may use and disclose your PHI for billing and payment purposes. For example:
- Submitting claims to your insurance company
- Collecting payment from you or a third party
- Verifying insurance coverage
For Healthcare Operations We may use and disclose your PHI for our healthcare operations. For example:
- Quality assessment and improvement
- Training and education
- Business management and general administrative activities
As Required by Law We may disclose PHI when required by federal, state, or local law.
Public Health Activities We may disclose PHI for public health purposes, such as:
- Preventing or controlling disease
- Reporting adverse events with medications or products
- Notifying persons of recalls
Health Oversight Activities We may disclose PHI to health oversight agencies for activities authorized by law, such as audits, investigations, and inspections.
Legal Proceedings We may disclose PHI in response to a court order, subpoena, or other lawful process.
Law Enforcement We may disclose PHI to law enforcement officials for certain law enforcement purposes.
To Avert Serious Threat We may disclose PHI to prevent or lessen a serious and imminent threat to a person or the public.
Military and National Security We may disclose PHI of Armed Forces personnel to military authorities. We may also disclose PHI for national security purposes.
Coroners and Funeral Directors We may disclose PHI to coroners, medical examiners, and funeral directors as authorized by law.
Organ Donation We may disclose PHI to organizations involved in organ procurement and transplantation.
Workers' Compensation We may disclose PHI for workers' compensation claims.
Uses and Disclosures That Require Your Authorization
For uses and disclosures not described above, we will request your written authorization before using or disclosing your PHI. This includes:
- Marketing purposes
- Sale of your PHI
- Most uses of psychotherapy notes
You may revoke your authorization at any time in writing, except to the extent we have already acted in reliance on it.
Your Rights Regarding Your Protected Health Information
Right to Access
You have the right to inspect and obtain a copy of your PHI maintained in a designated record set. Requests must be submitted in writing. We may charge a reasonable fee for copies.
Right to Amend
You have the right to request an amendment to your PHI if you believe it is incorrect or incomplete. Requests must be submitted in writing with a reason for the amendment. We may deny your request under certain circumstances.
Right to an Accounting of Disclosures
You have the right to request a list of certain disclosures we have made of your PHI. Requests must be submitted in writing.
Right to Request Restrictions
You have the right to request restrictions on certain uses and disclosures of your PHI. We are not required to agree to your request, except for disclosures to health plans for payment or healthcare operations when you have paid out of pocket in full.
Right to Request Confidential Communications
You have the right to request that we communicate with you about medical matters in a certain way or at a certain location. We will accommodate reasonable requests.
Right to a Paper Copy
You have the right to obtain a paper copy of this Notice upon request.
Right to Notification of Breach
You have the right to be notified in the event of a breach of your unsecured PHI.
Our Responsibilities
We are required to:
- Maintain the privacy of your PHI
- Provide you with this Notice
- Abide by the terms of this Notice
- Notify you if we cannot accommodate a requested restriction or confidential communication
We will not use or disclose your PHI without your authorization, except as described in this Notice.
Changes to This Notice
We reserve the right to change this Notice and make the new provisions effective for all PHI we maintain. The current Notice will be posted on our website and available upon request.

If you believe your privacy rights have been violated, you may file a complaint with:
